Women's Health participates with most major insurance plans. We recommend that you call ahead of time and check with your plan to determine whether we are in network. 

 

PLEASE REMEMBER:

  • It is important to bring your current insurance card to every office visit.

  • Copays are due at the time of service.

  • If required by your insurance plan, bring your referral from you Primary Care Physician.

  • You may be responsible for the portion of your charges not covered by your insurance policy.  As a courtesy to new patients we may ask that you get us insurance information ahead of time.  We will not only check to see if we are in network for you but also get a description of your benefits.  This helps us to better help you to understand what your out of pocket amounts may be prior to being seen.  If there are questions regarding that we are happy to visit with you.

Bring completed forms with you to visit

  • BRING the completed forms with you to your appointment.

  • It is important that you arrive 15 MINUTES PRIOR TO YOUR APPOINTMENT TIME.

  • You must bring your insurance card and driver's license (or other legal picture id).
